Please submit the names you would like included using the form below. Names may be submitted for inclusion in  just this year's book  for $36 (can be resubmitted yearly) or to be included in perpetuity (every year) for $360. 

Should you need to submit more names than the form allows, please submit additional forms.

The Book of Memory will be distributed at Yom Kippur services. If you will be attending only virtually please contact Andrea Shiflett  a.shiflett@bethahabah.org to obtain a copy.
